We will send you an intake form via email to complete (only takes 2 min). This will help us get your general availability along with going through our cancellation policy(*). It also requires a credit card(**) to secure the booking.
We book you in!
*We require 48 hours notice for any cancellations or changes to your appointment. Our goal is to provide quality healthcare to all our clients in a timely manner. When you book your appointment, you are holding a space in our calendar that is no longer available to other clients. Patients who provide less than 48 hours notice, or miss their appointment, will be charged the full visit to the card on file.
**This is more commonplace now with appointment-based services like ours, as we need a credit card on file to secure any appointments (in case of any last-minute cancellations as we do have limited spots + makes billing easier moving forward).
The credit card information is stored securely with Jane (the software we use for our appointments & billing) and is used by healthcare providers across Canada.
