What Is the Pelvic Floor and How Is It Affected Pre- And Postpartum?

Your pelvic floor is a group of muscles between your pubic and tailbone that support the uterus, bladder, and bowel. Because of its positioning, it’s greatly impacted by pregnancy.

As your pregnancy progresses and your baby continues to grow, the increased pressure on your pelvic floor can lead to intense pain and discomfort. This is especially noticeable in women with a weak pelvic floor, as they typically experience a greater deal of pain and uncomfortable side effects like incontinence and pain during intercourse.

Many women also suffer due to weakened pelvic muscles after childbirth and are left with many of the same issues affecting women prepartum. In some cases, the likes of pain and urinary leakage are even more profound, as the muscles have been stretched significantly during delivery.

What's involved in Pelvic Floor Physiotherapy Pre- And Postpartum?

During your pregnancy, most often in the second or third trimester, you may start to notice pain around your lower abdomen and difficulty controlling urinary and bowel movements. At this point, you’ll need to check in with your obstetrician or midwife, who will inform you whether you’ll need to consider starting physiotherapy. The same advice applies to women suffering these symptoms postpartum.

If physiotherapy is an option for you, you’ll first meet with one of your highly skilled therapists for a consultation. Throughout this first session, the therapist will evaluate the strength of your pelvic floor and your muscle’s ability to relax while also assessing additional issues like incontinence, pain during intercourse, and poor posture.

This will be done through a series of internal and external exams. Women attending postpartum will also discuss their birthing experience to help create a safe and individualized physiotherapy programme.

Physiotherapy can help avoid and overcome both pre- and postpartum pelvic floor issues by developing the strength of the muscle, increasing flexibility, and providing you with calming breathing techniques to loosen tense muscles.

Some of the central aims of pre- and postpartum physiotherapy for the pelvic floor include:

  • Improving muscle strength and tone
  • Minimizing pain and discomfort
  • Implementing effective breathwork
  • Enhancing the muscle’s flexibility and adjustability

Benefits of Pelvic Floor Physiotherapy Pre- And Postpartum

Physiotherapy allows women to enjoy their pregnancy and postpartum period without feeling uncomfortable, self-conscious, and tormented by pain. Some of the other benefits of physiotherapy for your pelvic floor include the following:

  • Physiotherapy for your pelvic floor not only prevents and eases pain in this area but can also reduce pain in your back and abdomen, as specific exercises can have a wide-ranging impact.
  • A weakened pelvic floor can lead to a myriad of associated issues, including bowel and urinary incontinence, sexual dysfunction, and prolapse. As your muscles improve in capability, tone, and function, your risk of suffering from these disorders lessens significantly.
  • The breathing techniques you’ll learn during your physiotherapy sessions work to relax and soften your muscles. These exercises do more than help you physically as they also work to put your mind at ease and de-stress, which is more important than ever pre- and postpartum.

Frequently Asked Questions

Many women begin prepartum physiotherapy for their pelvic floor during their second trimester. Starting at this time affords you the opportunity to build up the strength and tone of the muscles while the baby is still small and your pelvic floor isn’t under intense pressure.

Alternatively, some clients may prefer to start their sessions during their first or third trimester, as the right time for each woman can look different.

Once you’ve completed your first session in your house or condo, your physiotherapist will give you an indication of how many sessions are necessary. For many women, weekly appointments for around 6-8 weeks are sufficient, while others may need fewer or more periodic visits.

Prepartum pelvic floor physiotherapy can assist with a wide range of discomforts and side effects related to pregnancy, including:

  • Pelvic pain
  • Low back pain
  • Poor posture
  • Impacted mobility
  • Bladder and bowel control

It’s recommended that women who are around six weeks postpartum attend a physiotherapy session to check for any repercussions resulting from childbirth, regardless of whether you’ve noticed symptoms.

If your therapist is happy with your recovery progress, you may not need to attend any additional sessions. However, women experiencing pain, incontinence, or any other symptoms will need to schedule follow-up visits for several weeks until such symptoms subside.

Your pelvic floor makes up part of your core muscles, which means that many of the exercises you’ll complete throughout your postpartum physiotherapy sessions will strengthen your core area overall. Examples of such exercises include:

  • Glute bridges
  • Kegel exercises
  • Squats
  • Lunges
